We work closely with your primary care provider, whether he or she is located at one of our nine affiliated community health center sites, or outside of our network, to help you achieve the best possible health outcomes.
We offer inpatient and outpatient care, 24-hour emergency care, an Adult Day Care program, and physical therapy services, all based in Springfield, VT. For your convenience, medical oncology, cardiology, dermatology, and neurology services are also available locally at our specialty clinic located at Springfield Hospital.
